The Role: Brisbane South (Full time Mon-Fri)
- Field-based role, managing your own day on the road visiting various primary care clinics
- Delivering chronic disease education in an impactful and engaging manner to practice staff in primary care, including GPs, Nurses and practice managers.
- Support clinical staff on the appropriate identification of patients at risk through the use of their practice management systems.
- Developing a network in primary care
- Self-manage your appointments and your clinic accounts to help increase patient identification and management.

Through the identification of patients at risk and supporting clinics through the recall process, you will be helping facilitate high quality care within practice, improving patient outcomes and impacting patients’ lives.

The Ideal Candidate:
- A Registered Nurse (with current AHPRA number)
- Experience delivering education to primary care professionals (preferred, but not essential)
- Passionate about chronic disease management and serving patients
- Able to demonstrate strong commercial and business acumen
- Excellent communication skills
- Strong computer literacy/ software program skills
- Passionate, driven, positive, adaptable
- Full Australian driver’s license
